HOTEL FIRE SAFETY DIRECTOR to FIRE LIFE SAFETY DIRECTOR
Phase I: FSD (F-25/F-58) to FSD with Active Shooter (F-85)
- Attend the Actice Shooter/Medical Emergency Prepareness course and pass the school’s graduation exam
- Title of the F-85 C of F will read: FSD w/Active Shooter Prep.
- The expiration date on the FSD F-25/F-58 will be carried to the new F-85 card
Phase II: FSD with Active Shooter (F-85) to Temporary FLS Director (T-89)
For hotels withoutcomprehensive Fire Safety/Emergency Action plan
- Attend the EAP/non-fire course and pass the school’s graduation exam. (see note 2)
- Pass the Fire Department’s EAP/non-fire Computer Base Test at the FDNY (see note 2)
- Submit a letter from the hotel owner stating the status of Hotel’s Fire Safety /Emergency Action plan. (see note 1)
- After the FDNY reviews the documents, the Temporary FLS Director (T-89) will be issued.
Phase III: Temporary FLS Director (T-89) to FLS Director (F-89)
After the Hotel’s Fire Safety/Emergency Action plan has been accepted:
- After the Hotel’s Fire Safety/Emergency Action plan has been accepted, the T- 89 C of F holder will have to pass the EAP/non-fire onsite exam
- FDNY will issue a F-89 FLS Director C of F

- Do not wait to take your EAP/non-fire course and your Fire Department’s EAP/non-fire Computer Base Test
